2025-2026 School Year

A Message from Dr. Melissa Hocate:

As we prepare for a healthy and successful school year, we kindly ask for your support in ensuring all student health records are up to date. Please take note of the following important reminders:

Annual Physical Exams
All students are required to submit an updated annual physical exam. This helps us support your child’s well-being and ensure they are ready for the school year.

💉 Vaccination Records for New Students
If your child is new to our school, please provide a complete and current vaccination record as part of their enrollment documentation. There are 5 school required vaccinations which are only part of all vaccinations available to children which are generally complete by the age of 6 with TDaP booster before entry into middle school.

FACTS Medical Information Log in to your student’s FACTS portal and ensure that all medical and health information is current. This includes:

  • Any medical conditions or allergies
  • Prescription medications your student is currently taking
  • Permission for the school to administer over-the-counter medications

💊 Medication Policy
For the safety of all students, any medication that will be taken on campus must be discussed with the Health Coordinator in advance. A waiver and contract will be required for approved self carry/self administered medications after discussing with the Health Coordinator. This includes prescription and over-the-counter medications.

Upper School Student Athletes In accordance with CHSAA (Colorado High School Activities Association) guidelines, all students who plan to participate in any school sport during the academic year must have page 4 of a completed CHSAA waiver on file with the Athletic Director.

👉 Only page 4 of the CHSAA waiver needs to be submitted, found here.

While pages 1–3 may be used by a physician for the student’s physical exam, we strongly prefer that families use the CSS Annual Physical Exam Form, which can be found here.
