To comply with State and local regulations, The Colorado Springs School must maintain each student's current health and immunization records.
School physicals are current for a one-year period. Students must have a current physical exam form and an immunization certificate on file in the infirmary before the first day of school each year to attend classes and participate in sports. If a physical expires during the school year, an updated one must be submitted. To check when a student's physical exam expires, please contact Ann Runte, Infirmary Coordinator, at (719) 475-9747 or arunte@css.org.
Forms may be scanned and emailed to Ms. Runte or delivered in person to her office in the front of the Trianon.
Many of our mandatory medical and health records can be found below or in the FACTS Family Portal. Using their respective login credentials, parents are responsible for completing their child's medical paperwork. The following forms will require a physician’s signature:
Parents need to regularly update the Student Medical form and the Over-the-Counter Medications form in the FACTS Family Portal to ensure the school has the most up-to-date information.
Parents without health insurance can obtain coverage for their child(ren) here.
